Arts & Science Payroll Forms
To process academic payroll, we use a combination of PDF forms and electronic data entered into OASIS (Arts & Science Information System). Please note that OASIS is primarily used to appoint adjuncts, graders and course assistants each semester.
Three (3) types of PDF forms are used, depending on the nature of the payroll appointment. Once these PDF forms are completed they must be submitted electronically via our Google Form with supporting documentation to our office.

- Additional Appointment/Compensation Form (pdf)
- To add a new appointment to an employee or student who is already employed at the University

- Payroll Change Request (PCR) – aka Turnaround Document (TAD):
- To request changes to an existing appointment, such as a compensation increase or accounting change
- To obtain a PCR there are currently 3 methods, which depend on the employee and/or appointment type:

Email the faculty’s name and NYUID number to FAS Payroll (fas.payroll@nyu.edu). A PCR will be generated and emailed to you. Please complete and submit to FAS Payroll

A faculty member’s base salary should be charged to a grant for a course release and an accounting change is required

Labor Law 195
The NYS Department of Labor requires the following form(s) before or on the first day of work. These forms are in addition to the appointment letters that are required to be sent. Please add the applicable form(s) to your new hire paperwork for all Academic and Student appointments (codes 102, 103, 112, 113, 115, 118, and 119).
